Conservative Treatment in Clubfoot Deformity

چکیده

Treatment of the congenital clubfoot (CCF) starts its development in the early 20 century and is still continuing today. Surgical techniques provide functional and radiographic data, but the subsequent later data of poor function and recurrence of deformity gave grounds the overall therapeutic process to be reconsidered in the middle of the last century. There are two methods that are generally accepted as proved to be effective in clubfoot treatment. In 1939 Kite introduced a method to correct the clubfoot by using a series of plaster casts. Two decades later his follower – Ignasio Ponseti. (1968) modified the method, developed and completed it. Regardless of literature evidence on significantly better results after the Ponseti treatment, Kite’s method is still popular and used in some isolated countries in the world. Knowledge of both methods as stages in the development of theory and practice in the correction of the congenital clubfoot along with modern understanding of the etiology and in particular of the pathobiomechanics of the condition, create a positive outlook for improvement in the prognosis and treatment outcomes of this disease. Level of evidence: Second Level, systematic overview.

The incidence and treatment of rocker bottom deformity as a complication of the conservative treatment of idiopathic congenital clubfoot.

Rocker bottom deformity may occur during the conservative treatment of idiopathic congenital clubfoot. Between 1975 and 1996, we treated 715 patients (1120 clubfeet) conservatively. A total of 23 patients (36 feet; 3.2%) developed a rocker bottom deformity. It is these patients that we have studied. The pathoanatomy of the rocker bottom deformity is characterised by a plantar convexity appearin...

Clubfoot: the treatment outcome using quantitative assessment of deformity.

ABSTRACT INTRODUCTION The recent trend in management of congenital idiopathic clubfoot tends towards conservative treatment. This study reviews the outcomes of treatment in our practice using the quantitative clubfoot assessment of the deformity (QCAD). METHODS Thirty patients (38 cases of clubfoot) with congenital idiopathic clubfoot treated at Sarawak General Hospital were followed-up fo...

Selective soft-tissue release for recurrent or residual deformity after conservative treatment of idiopathic clubfoot.

We reviewed the results of a selective à la carte soft-tissue release operation for recurrent or residual deformity after initial conservative treatment for idiopathic clubfoot by the Ponseti method. Recurrent or residual deformity occurred in 13 (19 feet) of 33 patients (48 feet; 40%). The mean age at surgery was 2.3 years (1.3 to 4) and the mean follow-up was 3.6 years (2 to 5.3). The mean Pi...

FOREFOOT ADDUCTION IN CHILDREN. Management and Treatment.

Forefoot adduction is a common condition between metatarsus adductus, Z-shaped foot and residual clubfoot. This deformity is located in a pure transverse plane at Lisfranc’s joint. Isolated metatarsus adductus is corrected spontaneously for the majority of newborns. In rare uncorrected cases, it could result in Z-shaped foot with a functional hindfoot valgus to equilibrate the resistant metatar...

Functional Results after Surgical Correction of Neglected and Relapsed Clubfeet

Introduction: Clubfoot is a common congenital deformity. Neglected clubfeet are not uncommon in the developing world where many children are brought late for initiation of treatment. The response to conservative treatment may not be adequate so as to achieve correction of deformity in all cases. Hence, some of these cases may require surgical treatment for correction of deformity. International...
